What are the responsibilities and job description for the Adult Mental Health Practitioner position at Zumbro Valley Health Center Inc?
We are looking for a caring, and devoted Mental Health Practitioner to join our growing team of professionals! As a Mental Health Practitioner you will provide direct mental health services and work one-on-one with clients out in the community, at their home, or at the office. Responsibilities :
Maintain a caseload of clients.
Help develop and work will all members of the treatment plan team as often as needed.
Facilitate services to clients individually and in a group setting.
Coach, teach, educate, and model independent living skills to clients.
Coordinate and make further referrals for clients as needed.
Seek treatment consultation from Clinical Supervisor.
Accurately complete all required documentation and notes for each client in a timely manner.
Requirements & Qualifications
Bachelors Degree in Social Work, Psychology or a related human services field.
1 year of experience providing services to individuals with a mental illness or substance use disorder required.
Must qualify as a Mental Health Practitioner in the State of MN
Licensed as a Social Worker in the State of MN preferred.
Provisional License in the State of MN will be considered.
Experience working with severe emotional disturbances in adults is preferred.
Valid driver's license and proof of auto insurance.
Pay : $24.37 - $29.24 per hour. Compensation is based on years of experience.
