Zumbro Valley Health Center is seeking a hardworking, and passionate Creek Targeted Case Manager to provide services to clients at The Creek and Olmsted County. As a Creek Targeted Case Manager, you will have a combined role providing intensive case management services to clients at the Creek and traditional case management to clients in Olmsted County. The Creek is a 12-unit, 90-day emergency shelter that focuses on guiding clients with a clear path to long-term housing.
Responsibilities
- Maintain a caseload of clients.
- Coordinate and make referrals for client services that will help the individual meet their goals.
- Complete and accurately update all documentation as required.
- Work collaboratively with all members of the treatment plan as often as needed.
- Follow-up on referrals and monitor the clients progress.
- Provides crisis assessment and intervention services to clients as needed.
- Provide constant empowerment and support to clients to assist with achieving long-term housing.
- Work collaboratively with Olmsted County and Catholic Charities to ensure clients maintain a safe, clean and welcoming environment.
Qualifications
Requirements & Qualifications
- Bachelor's Degree in a Social Work, Psychology, Sociology or a Behavioral Science field.
- Minimum of 1 year of experience working in the mental health field or providing services to those unhoused required.
- Valid Driver's license and Proof of Auto Insurance required.
- Must be creative and innovative in finding ways to get needs met for the individual.
- Must have the ability to work with individuals who have complex symptoms.
- Knowledge of case management services and community resources in Rochester area.
- Must be able to build trust and rapport quickly.

Pay: $24.37 - $29.24 per hour. Compensation is based on years of experience.
