Description
We are looking for a Full-time Housing Case Manager to provide Housing Support and Case Management services to individuals that have a history or are currently experiencing homelessness. This position requires a compassionate, resourceful, and highly organized individual who can effectively collaborate with a team to help empower clients to achieve stability and independence.
Responsibilities:
- Coordinate, make referrals, and follow up for services and resources as needed.
- Complete all admission and discharge paperwork in a timely manner.
- Attend and participate in case consultations, and supervision meetings.
- Assist clients with securing housing, and interface with landlords to maintain housing.
- Provide support services once housed to help improve stability and longevity in housing.
- Use motivational interviewing techniques and engagement strategies to build an alliance with clients.
Qualifications
Qualifications
- Bachelors Degree in Social Work, Psychology or a related human services field.
- 1 year of experience providing services to individuals with a mental illness or substance use disorder required.
- Must qualify as a Mental Health Practitioner in the State of MN
- Licensed as a Social Worker in the State of MN preferred.
- Provisional License in the State of MN will be considered.
- Experience working with severe emotional disturbances in children is preferred.
- Valid driver's license and proof of auto insurance.
