Zumbro Valley Health Center is seeking a full-time Housing Support Practitioner. The Housing Support Practitioner will work directly with individuals and families experiencing homelessness, helping them navigate available resources, secure housing, and achieve long-term stability. The ideal candidate will have knowledge of mental health and chronic substance use disorders and of community resources. This position requires a skilled communicator with strong problem-solving abilities and a desire to empower individuals to succeed.
Responsibilities
- Interface with landlords to secure housing for individuals experiencing homelessness.
- Assist in identifying resources related to basic needs for housing maintenance.
- Provide supportive services once housed to help improve stability and longevity in housing.
- Screen client for eligibility and complete admission process.
- Use motivational interviewing techniques and engagement strategies to build an alliance with clients.
- Assist clients with household management responsibilities.
- Discharge clients according to policies and procedures.
Qualifications
Requirements & Qualifications
- Bachelors Degree in Social Work, Psychology or a related human services field.
- 1 year of experience providing services to individuals with a mental illness or substance use disorder required.
- Must qualify as a Mental Health Practitioner in the State of MN
- Licensed as a Social Worker in the State of MN preferred.
- Provisional License in the State of MN will be considered.
- Experience working with severe emotional disturbances in adults is preferred.
- Valid driver's license and proof of auto insurance.
Pay: $24.37 - $29.24 per hour. Compensation is based on years of experience.
