Zumbro Valley Health Center is seeking a part-time Mental Health Professional to work with our Case Management, ARMHS, and Peer Support programs.
Responsibilities
- Reviewing and signing off on documentation
- Coordinate review of clinical records to assure that internal and external standards are met.
- Track admissions to ensure that required documentation is completed within designated timeframe.
- Determine eligibility for rule 79 case management and complete the state eligibility form, as needed.
- Review Commitment, Guardianship and Vulnerable Adult reports.
- Provide treatment supervision, direction and oversight for each client served.
Qualifications
Requirements and Qualifications
- Master's in Counseling, Psychology, Marriage & Family Counseling or Social Work.
- Independent professional licensure in the State of MN (LP, LPCC, LMFT, or LICSW) required.
- Must meet the MN State Statute as a Mental Health Professional.
- Two years of experience working in a multi-disciplinary mental health setting is desired.
- Valid Driver's License and Proof of Auto Insurance
