Zumbro Valley Health Center is seeking a Mobile Crisis Practitioner. The Mental Health Practitioner is available for mental health crisis assessment, intervention and stabilization on weekdays from 8am to 5pm.
Responsibilities
- Creates a crisis plan in conjunction with the Mental Health Professional and the client who has been referred.
- Completes a risk assessment of individuals current state to be reviewed by Mental Health Professional.
- Documents care coordination efforts to ensure the individuals safety and service needs are met.
- Provides necessary skills based education to clients in the community to ensure mental health stabilization.
- Maintains communication with Mental Health Professional during crisis intervention as appropriate.
- Demonstrate strong emergency clinical decision-making skills.
- Utilize verbal crisis de-escalation techniques
- Assures clinical documentation is accurate and completed before leaving the intervention/stabilization site.
- Is accessible via cell phone and responds to all incoming crisis calls during scheduled hours
- Responds to crisis calls by arranging face to face meeting that may occur in the individual's home, community setting, or emergency room.
Qualifications
Requirements and Qualifications
- Bachelor's degree in social work, psychology, or a related human services field.
- One year of experience providing services to individuals with a mental illness or substance use disorder required.
- Must qualify as a Mental Health Practitioner in the State of MN
- Valid driver's license and proof of auto insurance.

Pay: $24.37 - $29.24 per hour. Compensation is based on years of experience.
