The Care Coordination Supervisor oversees the daily operations and staff of the Care Coordination program, which includes Care Coordination, Enhanced Care Coordination, and Community-Based Care Coordination. This role ensures adequate program coverage, adherence to timelines, and achievement of productivity and performance metrics. The supervisor is responsible for assisting with program development and assessment, which includes research, meetings with stakeholders, and data analysis.

We are one of the few providers in the state, to provide such an integrative and comprehensive care to all children, adults, and families. In order to provide an integrative approach we offer a wide variety of services including Outpatient Psychotherapy and Substance Use, Inpatient Residential Treatment, Withdrawal Management and Detoxification, Housing, Skills – Based Training and Targeted Case Management for adults and children, and more.
Pay: $31.87 - $39.05 per hour. Compensation is based on years of experience.

Description

Responsibilities
- Hire, orient, supervise, address performance concerns, and conduct performance reviews.
- Ensure adequate staffing and daily program coverage.
- Attend and participate in clinical and administrative meetings as directed.
- Organize and lead care team meetings and huddles.
- Maintain comprehensive knowledge of agency programs, community resources, and support services; disseminate knowledge across the agency as indicated.
- Assist in developing and implementing program policies and procedures.
- Monitor referrals, delegate client outreach tasks, and ensure timely follow-up.
- Serve as a resource liaison for team members on forms, community contacts, and resource information.
- Conduct quality assurance reviews and ensure compliance with data privacy, HIPAA, and agency policies.
Requirements & Qualifications
- Bachelors degree in Social Work, Psychology or related field required.
- Must qualify as a Mental Health Practitioner.
- Minimum of two years of experience working with adults and/or children with behavioral health disorders.
- In-depth knowledge of agency programs and services offered.
- Valid driver's license and proof of auto insurance.
