Location BBC Bronson Battle Creek Title Employee Assistance Program Case Manager Responsible for the delivery of employee assistance program (EAP) intake screening and assessment, consultation, and solution-focused support services to all EAP members of diverse backgrounds and populations. Provides clinical screening and assessment in such areas as mental health, couples/family, depression/anxiety, trauma, suicide, interpersonal violence, children to adults, abuse/neglect, grief, crisis intervention, advocacy, and substance abuse. Provides in-the-moment support and crisis stabilization to high-risk clients, case management and triage, and referral coordination with affiliate providers within our national network. Is responsible for providing direct patient care. Must demonstrate competencies specific to the population served. Additional Job Description Master's degree in social work, counseling, psychology, or related field required Licensed Master’s Social Worker–Clinical, or Licensed Professional Counselor, Limited License Psychologist or Temporary Limited License Psychologist credentials required (must be in good standing and valid in Michigan). Individuals with EAP experience preferred 3 years full-time professional level work experience in mental health with all age groups, with preference for individuals with a background in treating substance use disorders Responsible for answering calls in the Bronson HelpNet (EAP and Behavioral Health Specialists) call center, conduct mental health screenings, provide empathic, nonjudgmental emotional support, and meeting caller needs. Assist clients in locating and securing appointments for counseling within national network of providers, in coordination with insurance benefits. Maintains confidential records and files for Bronson HelpNet offices, including highly confidential materials related to contract and client activity Develop and review individualized clinical care plans and case management services for management referral clients in collaboration with affiliate providers to address presenting issue(s). Performs clinical assessments with client and/or eligible members to determine appropriate level of care and type of intervention to formulate treatment and discharge planning case management Performs provider relations responsibilities in collaboration with Information Specialist, Intake Team, Leadership and credentialing agency, including provider Agreements, provider development and network coverage, documentation and quality of work, communication, provider engagement, affiliate referral process Communicates and coordinates services provided with affiliate providers and corporate clients, within HIPAA (Health Insurance Portability and Accountability) regulations and guidelines. Work between the Battle Creek and Kalamazoo HelpNet sites Participates in on-going education and training to meet professional and program requirements. Participates in required in services, staff meetings, committees, and completes all required competencies. Represents HelpNet and Bronson to the community by performing various functions i.e., speaking engagements, networking, and educating about program services. May Perform minimal psychotherapeutic interventions in a variety of modalities, including individual, group, couples, and families. Shows commitment to providing outstanding customer service to all customers reflective of our mission, values, and customer service standards. Maintains a working knowledge of applicable accreditation standards, Federal, State, and local laws, and regulations. Considerable skills, knowledge of/and ability to be empathic, crisis and clinical assessment, intervention, and counseling techniques, as well as equitable care to all clients and customers Considerable ability to communicate effectively with individuals experiencing emotional crisis or emergency situations in accordance with Hospital policies and procedures Knowledge of community service organizations and the types of assistance they provide Ability to communicate effectively in both oral and written form. Proficient in Microsoft Word, Excel, EMR (Electronic Medical Record) and skilled in various technology programs Ability to establish and maintain effective relationships with corporate customer representatives (Human Resources), referral and community service agencies, providers, and clients Performs related duties as assigned.
