Job SummaryUnder the direction of the Wraparound Supervisor, the Wraparound Facilitator carries a caseload of children and/or adolescents and their families. The Wraparound Facilitator conducts weekly meetings with families to assess progress toward their goals and communicates progress or barriers to treatment to the referring party. The Wraparound Facilitator frequently communicates and consults with team members in developing and maintaining a Plan of Care and Crisis Safety plan for families. They are well-versed in and are able to communicate to others the Wraparound philosophy, principles and process. The Wraparound Facilitator is responsible for completing all required documentation in accordance with agency and County standards.
Essential Functions
  • Responsiveness to the families’ identified needs
  • Development of family-centered help-giver skills
  • Achievement of professional and family partnerships
  • Incorporation of informal supports, natural family helpers, and other community resources
  • Outcomes for child and families
  • Feedback from children and families community services providers
  • Responsible for initial enrollments
  • Maintain contact with referring party and communicate family’s participation, progress or barriers to treatment
  • Collaborate with outside agencies to provide services needed for families
  • In collaboration with team members, create and maintain Plan of Care and Crisis Safety Plan for families
  • Conduct weekly meetings with families to assess their needs and progress toward goals
  • Maintain Wraparound chart in accordance with HIPAA, DMH, DCFS, and Los Angeles County standards
  • Complete and submit all required paperwork, notes and reports including but not limited to:
    • Department of Mental Health progress notes
    • Daily contact notes
    • Incident reports
    • Suspected child abuse reports
    • Monthly status reports
    • Available to respond to crisis 24 hours a day, 7 days a week
    • Respond to crisis and coordinate immediate intervention to stabilize family
    • Consult with team members and supervisor on a regular basis regarding caseload and appropriate course of action in crisis situations and incidents
    • Leads the individual Child Family Team by:
      • Following the four phases of Wraparound and all the activities identified
      • Ensuring the principles of Wraparound are adhered to by all team members
      • Ensuring that all the strengths and needs are identified
      • Ensuring all the identified services are provided in a timely and appropriate manner
      • Being the contact point for children, families, service providers and the community
      • Ensuring the County representatives has adequate opportunities for input and access to the team and planning process
      • Assess the need to request flex funds and communicate need to immediate supervisor
  • Regularly meet with supervisor and/or clinical staff
  • Report any concerns regarding client services and satisfaction to appropriate team member
  • Attend all program or agency trainings and meetings as required
  • Drive personal vehicle as necessary
      • Sensitive and responsive to the cultural differences among the agency’s client population
      • Actively participate in the quality improvement project
The Wraparound Facilitator will take on special projects or additional duties as assigned by the Wraparound Supervisor and/or Wraparound Director.
Competency Requirements
  • Ability to speak, read, write and understand English
  • Ability to work effectively as a member of the Wraparound team
  • Ability to successfully work with families of diverse ethnic and cultural backgrounds
  • Ability to assess a crisis situation and respond in a timely and professional manner
  • Must have excellent oral and written communication skills
  • Must have excellent interpersonal skills
  • Must have strong organizational and planning skills
  • Knowledge of social services system and community resources
  • Knowledgeable in Wraparound philosophy, policy and process
  • Demonstrate skill in operating Microsoft Word, Outlook and Excel
  • Skilled in relationship building, conflict mediation and facilitation a plus
  • Bilingual (English/Spanish) preferred
Education And Experience
  • Bachelor’s degree in Social Work, Sociology, Psychology or related field and 1 year of relevant experience preferred
  • Experience in crisis management
  • Experience with Wraparound preferred
Certifications/Licenses
  • Valid CA driver’s license
Additional Requirements
  • Tuberculosis clearance
  • Department of Justice/Fingerprint clearance
  • Satisfactory completion of health screening
  • Proof of legal eligibility to work in the United States
  • Satisfactory driving record

Physical DemandsWhile performing the duties of a Wraparound Facilitator, the employee is frequently required to walk, drive, and use their fingers to manipulate a keyboard or writing instrument. The employee is occasionally required to site, stand and reach. The employee is rarely required to run, kneel, and lift more than 15 pounds.
Work EnvironmentThe work environment is indoors and outdoors. The noise level varies from moderate to loud. The work environment is community-based and requires conducting home visits.
