Part-time w/ Benefits (scheduled 60-71 hrs per pay period) - Day Job Description SCOPE OF POSITION The Diabetes Wellness Coach is responsible for providing a safe environment for participant to explore their health and lifestyle choices. The Coach will use motivational interviewing skills to review patients' goals and will collaboratively develop healthcare meal, exercise or wellness plans tailored to each patient's needs. The Coach will provide recommendations to reduce or eliminate unhealthy or high-risk behaviors. The Coach will use the diabetes standards of care, and follow the AADE 7 guidelines to lead and empower the patient in diabetes self-management and will consult with the RN Diabetes Programs Manager as needed. The Diabetes Wellness Coach is also responsible for exhibiting effective communication skills while working with patients, families, and other healthcare providers. The Coach must demonstrate the ability to use motivational interviewing to identify barriers and behaviors that will allow them to reach the highest standard in coaching and education. The coach must have the expertise to focus on improving exercise and physical fitness for the participant. Must have excellent telephone etiquette, be flexible with schedule and be willing to participate in continuing education and workshops that will be offered periodically. Must be accepting of new ideas, concepts, and approaches to providing education to patients. REPORTING RELATIONSHIP See Table of Organization. EDUCATION AND EXPERIENCE Bachelor of Science in Exercise, Sport and Health Education, preferred Registered Nurse or Registered Dietitian with a minimum of 3 years of experience.
