Job Scope

1. Provide psychosocial care and case management under the guidance of the Medical Social Worker.
2. Assess and process financial assistance applications for patients and their families.
3. Conduct home visits and assess living conditions for follow-up by the Medical Social Worker.
4. Prepare brief social assessments for continued case management.
5. Support group work sessions, including logistics coordination and co-facilitation.
6. Participate in community mapping and engagement efforts.
7. Assist in planning and execution of community events.
8. Contribute to departmental education, quality improvement projects, and professional ethics adherence.

Job Requirements

1. Minimum GCE ‘O’/’A’ Levels or NITEC in Community and Social Services.
2. At least 2 years of relevant experience in social services is an advantage.
3. Strong interpersonal and communication skills to engage with patients and families.
4. Ability to work independently and collaborate with multidisciplinary teams.
5. Organized, proactive, and committed to improving patient outcomes.
