URGENT* Home Care Nurse (Palliative Care, Office Hours, Islandwide) #HRY

Responsibilities

• Make home visit to patient home
• Provide chronic disease, activities of daily living and/or palliative advice and counselling, as well as nursing procedures such as nasogastric tube insertion, catheter management and wound care.
• Write case report and update sytem on patient condition
• Liaise with relevant personnels and tap into resources to provide for patients' needs
• Provide emotional support for patients and family members

Job Requirements

• Min. Diploma/ Degree in Nursing
• Registered licence with Singapore Nursing board (SNB)
• Specialised Diploma in Palliative Care
• Min. 3-5 years of relevant perm acute wards / hospice experience in gerontology and / or palliative care preferred
• Ability to work both independently as well as in a multidisciplinary team, families and caregivers
• Service oriented with good communication skills and a love for clinical care delivery.
