Job Purpose
The social worker will provide case management to clients and their families, which include social and financial assessment. To provide referrals to community partners such as SSO and religious organisations.
Develop casework intervention plans for assisting families with important decisions about their loved ones such as Advance Care Planning.
Develop Individual Care Plan with the Multi-Disciplinary Team.
Introduce Care-giving Training for all new and existing Care-Givers.
Form at least one support group for Care-givers who may face caregiving issues
Duties and Responsibilities
Accessing the various referrals from AIC and Cares System. Review all applications from AIC. Ensure a pre-assessment is conducted within a stipulated timeframe.
Ensure the social work associate provides the necessary admin support during intakes such as processing of applications, collection of deposits, service agreements are endorsed and all documents are uploaded in Cares.
Following up and checking on programme fees are submitted to Finance.
Collaborate with OT/PT on scheduling for rehab clients
Coordinate with drivers on Transportation arrangements.
To conduct surveys with the clients such as OQPOL (monthly) and satisfaction surveys (twice a year. ie June and December)
Identify cases to put up for Medifund /Financial assistance
Build rapport with clients and their families. Maintain constant communication with clients while addressing their concerns and goals and helping them with their issues.
keep positive attitudes. Provide guidance to families on psychosocial needs of the clients and ways they can access internal and external resources.
Risk assessment - assessing risk of self-harm (e.g. suicide) and to others (e.g. family violence, elder abuse).
Provide casework management for our clients and coordination with external agencies such as hospitals and government agencies who may require financial assistance for their medical appointments.
The Develop and implement individual care plans (ICP) for clients and provide therapeutic support as needed. Working with other multidisciplinary team (MDT) members.
Execute the ICP and monitor progress. Develop and implement programmes and activities through a planned/structured manner.
Interpret OQPOL surveys and incorporate the findings for each client’s ICP. Respond to areas/items that clients have given feedback for satisfaction surveys.
Evaluate existing, new and prospective clients based on their needs, desires and limitations.
Update clients particulars and social report/review. Conduct casework evaluation, maintain case notes/documents.
To refer clients and/or families to counsellors for clients who suffer from poor mental health states (e.g. depression, anxiety), and coping and adjustment difficulties in their transition.
Financial assessment and prepare periodic reports as required - identifying and referring cases for financial assistance.
Any other additional tasks assigned by the Management.
Qualifications & Skills Required
A Degree in Social work, Registered Social Worker
At least 4 years’ social work experience
Prior experience in residential setting for the elderly
Have good knowledge of government financial schemes
Ability to work with different groups, elderly, stake-holders
Able to speak local dialets
Excellent written and inter-personal communication skills
Generate reports and statistics as required by MSF
