South Central Community Family Service Centre (SCC) provides support to the low-income and vulnerable families in our community. While giving support to families who seek help in financial, social or relationship issues, we encourage community self-help and cooperation.

About The Role Of Transformation Specialist / Social Worker
Are you passionate about transforming lives and creating lasting impact? As a Transformation Specialist, you will play a key role in helping low-income families break the cycle of poverty and achieve their dreams. This is a chance to empower and uplift families, guiding them towards long-term financial independence, homeownership, and social mobility.
In this role, your work will be focused with families who generally present with more stability (i.e. minimal risk factors) but face barriers or require support to achieve social mobility. The Transformation Specialist is responsible for facilitating change and improving the lives of individuals, families, community facing the challenges of poverty through the provision of specialist interventions including but not limited to social work, home ownership, and livelihood interventions to realise their aspirations of getting out of the poverty cycle. Your interventions will help them overcome these obstacles and achieve long-term, meaningful progress. You will be part of a dynamic, supportive team dedicated to long-term family transformation and positive community impact.
Job Description
- Work collaboratively with community and ecosystem partners to empower and enable socially disadvantaged low-income families to achieve long-term goals like homeownership (with a focus on maintaining it for at least six years) or financial stability and asset-building, enabling them to create a brighter future and escape the poverty cycle.
- Provide holistic casework interventions, focusing on critical areas such as homeownership, livelihood improvement, and debt management. Assess individual family needs, offer guidance, and equip them with the knowledge and skills necessary to achieve sustainable change.
- Maintain accurate and confidential family records, documenting progress, interventions and outcomes, on the required platforms such as (but not limited to) Social Service Net (SSNet), CaseConnect, and Family Transformation Management System (FTMS). Your attention to detail ensures that every family’s journey is well-documented, and the positive changes are measurable.
- The focus of this role is for families with a specific profile: low to no risk factors (e.g. family violence or suicide risk etc.) who still require professional intervention to address the barriers of poverty.
Job Requirement
- You should possess a Degree or Post Graduate Diploma in Social Work from recognised universities; with a pathway to work towards being an Accredited Social Worker
- You should be a passionate change-maker who is driven by a deep desire to make a difference in people’s lives. You believe that every family has the potential for transformation, and you are committed to guiding them along that journey.
- You should be a creative problem-solver who is innovative and resourceful in designing tailored solutions that address the unique challenges faced by each family. You excel in collaboration and know how to connect families with the right resources and support systems.
- You should possess the ability to communicate well, both verbally and in writing, and be able to empathize and allow families to feel empowered and supported, with a commitment to building healthy and strong relationships with the families you serve
