Medical Social Worker - WC03

Roles and Responsibilities:

• Referral and Stakeholder Management:
  Engage partners for referrals and manage relationships with stakeholders.
• Patient & Caregiver Support:
  Assess, create care plans, and track patient progress. Provide counseling and connect to appropriate services.
• Financial Counseling:
  Assist with admissions, treatment costs, and guide patients on financial options (e.g., insurance, Medisave).
• Patient Visits:
  Conduct home, hospital, and dialysis center visits for assessments and counseling.
• Advocacy:
  Protect patient welfare and rights.
• Collaboration:
  Work with healthcare professionals and social agencies for patient care.
• Support Groups:
  Organize support programs for patients and caregivers.
• Process Improvement:
  Review and improve care processes.
• Discharge/Transfer Facilitation:
  Assist with patient discharge and transfer.
• Other Duties:
  Perform tasks assigned by the department head.
