My Health Team Facilitator

My Health Teams is built around strong partnerships. As a member of the team, you will work to help patients and
clients get the care they need, when they need it. The primary goals include improving access, providing safe, quality
care, increasing focus on patient-centered care, connecting care providers to provide seamless transitions in care and
improving efficiency and supporting sustainability of the health system. The My Health Team Facilitator is responsible
for leading the development of the My Health Team initiative within the Northern (geographic) Region. The incumbent
will assist the inter-professional team in achieving identified My Health Team goals as well as other provincial priorities.
The incumbent will work in collaboration with key stakeholders from Opaskwayak Health Authority (OHA) and Northern
Regional Health Authority (NRHA). As the Northern My Health Team develops, additional partners may join.

This position requires service and travel to outlying communities within the region and the province.

QUALIFICATIONS

• Post-secondary degree in a health-related discipline

• Experience in project design, management, implementation and evaluation

• Other combinations of education and experience may be considered

• Current active practicing registration with applicable regulatory body

• Valid Manitoba Class V driver’s license, access to a vehicle, and willingness to travel the region year round

• Working knowledge of Windows based programs (Microsoft Word, Excel, Access, PowerPoint, Outlook) and

• Knowledge of the principles of Primary Health Care, Population Health Promotion, and Shared Health’s

• Knowledge of the regional health authority, primary care clinics physician environments, and community

• Knowledge or experience with electronic health solutions (i.e. Accuro EMR) and EMR optimization

• Successful candidate subject to provide security checks in accordance with Security Checks for Employment
