Salary: $52,000 annually
Position: We’re looking for a Twin Cities based direct care case manager that loves working with youth to join our Supportive Services Department! Skilled in trauma-informed care, Mobile Case Managers help youth and young adults work to secure community resources, and plan for the next step on their journey. The Safe Harbor Regional Navigator program is housed at and supported by 180 Degrees’ and operates independently to ensure equity and value to the entire network. This role will work in service to Region 8 (East Metro) that includes Ramsey, Anoka, Dakota, Isanti, Chisago and Washington Counties all in Minnesota. This is a large, diverse geographic area, including urban and suburban communities, with some rural-feeling areas as well. The main responsibility is to serve the Safe Harbor Regional Navigator by serving a case load of youth that have been sexually exploited.

Care Coordination Responsibilities:
1. Support the Regional Navigator in building effective community partnerships.
- Maintain direct care relationships with designated stakeholders in human services, perform ongoing outreach activities as requested, and attend meetings or conferences as guided by the Regional Navigator.
- Consult on identifying gaps in services and implementing an action plan.
- Ensure timely and accurate documentation to support the data collection needs of the Regional Navigator.
- Support the Regional Navigator in delivery or creation of trainings as needed.
2. Directly serve sexually exploited youth by managing an ongoing caseload of up to 15 youth.
- Provide direct care face-to-face, over the phone, and virtual case management for youth in the community.
- Coordinate with shelter Case Manager(s) for successful entry into and/or transition out of shelter.
- Support youth in the community to access human services resources they need to be successful.
- Foster relationships with community-based providers that offer services to our target audience.
- Provide backup assistance for the 24/7/365 helpline service in the absence of the Regional Navigator.
- Provide youth group work related to sexual exploitation as guided by the Regional Navigator.
- Regularly communicate with the Regional Navigator regarding community-based youth that need referral to in-shelter services.
3. Actively participate in the Supportive Services department at 180 Degrees.
- Fulfill individual funding service requirements and role expectations.
- Align all services with best practices to effectively serve youth.
- Collaborate with the Regional Navigator to address human services support needs, recommend referrals, seek consultation, and establish new processes as necessary.
- Engage in the preparation and attendance of routine case consultations, ensuring inclusion of relevant community partners and shelter staff while respecting data privacy considerations.

Human Service Competencies:
- Knowledge of sexual exploitation, trauma responsive care, youth-work, and case management best practices, Safe Harbor knowledge preferred.
- Awareness of one’s own cultural identity, with an understanding of how bias and experience influences their work with diverse individuals.
- Thrives on direct engagement with youth, employing skills in motivational interviewing, de-escalation, and conflict resolution.
Qualifications:
- Minimum age: 21
- Two years of case management experience related to youth experiencing or at-risk of sexual exploitation or trauma services preferred.
- Bachelor’s degree in public health, social work, human services or related field preferred.
- Experience in the nonprofit sector preferred.
- Nonprofit experience with grant adherence and reporting knowledge, preferred in Safe Harbor or related grants.
- Dependable transportation for in-person work; reliable internet and phone connection for remote hours.
- Valid driver’s license and insurance.
- Ability to clear a Department of Human Services background screening.
- Lived experience in place of certain credentials will be considered for all positions.
