Job Overview
As a Community Navigator, you'll play a pivotal role in empowering individuals with developmental and/or physical disabilities. This position involves working closely with participants, their support teams, and service providers to develop Person-Centered goals and objectives. You'll facilitate meaningful community engagements, ensuring each participant’s safety and personal growth.
Key Responsibilities
- Conduct assessments to determine participants' interests and needs.
- Coordinate with a diverse team to support participants' integration into the community.
- Advocate for participants, ensuring they have access to necessary services and support.
- Develop, implement, and monitor community-based programs that align with participants' Person-Centered Plans.
- Serve as a liaison between participants, their families, and community resources.
- Lead and support staff in delivering high-quality, person-centered services.
Qualifications
Qualifications
- Deep understanding of learning methods and services for adults with disabilities.
- English and Spanish bilingual preferred.
- Strong case management skills and experience in community program development.
- Exceptional leadership, team building, and interpersonal skills.
- Proficiency in personal computer use and relevant software applications.
- Ability to work independently, make well-informed decisions, and manage multiple projects.
- Bachelor's degree or equivalent in a related field preferred.
Working Environment
- Open, comfortable office setting with occasional community travel.
- Dynamic and supportive team atmosphere.
Work Schedule
Full-Time Position, Monday through Friday
