What are the responsibilities and job description for the Group Home / Residential Manager - Foothills position at Abound Health?
We are looking for a FULL-TIME Residential Manager that will provide direct care along with supervisory and household maintenance in a Group Home setting.
RESPONSIBILITIES:
- Supervision/Coordination: This position is responsible for the day-to-day operation of a DDA group home and an apartment program. This involves direct supervision of habilitation technicians; ensuring adequate staff coverage when scheduled staff are sick or on vacation; organizing the daily routine for staff and residents; participate in on call duties as assigned; ensuring rules and regulations governing the operations of a DDA group home or apartment program are followed; participation in the recruitment, evaluation, disciplinary action, etc. of habilitation technicians; exercise sound judgement in potential crisis situations ensure safety measures are taken to prevent harm or injury to residents or staff; review daily documentation of staff; plan and support community activities; participate in admission committee and habilitation planning for residents; participate in treatment team meetings; provide direct care in the absence of the other habilitation technicians; arrange and/or ensure medical, dental, DSS, Social Security, etc. appointments are arranged and maintained for each client; coordinate regular purchases for the home groceries, janitorial supplies, etc.; coordinate in conjunction with the residential services supervisor, maintenance and repair of equipment, facility, appliances, vehicle, etc.;
- Habilitative Services: Implementation of habilitative, behavioral, social, medical, nutritional, vocational outcomes per individual client Treatment/Habilitation Plan written by and overseen by a QP/ case manager/psychologist/OT/PT/ST. Outcomes implemented may include, but not be limited to, self-help (i.e. toileting, hand washing, eating, etc.); leisure (i.e. playing simple table games, tossing ball, playing basketball, going to movie, eating in restaurant, etc.); home living (i.e. meal preparation, making a sandwich, cooking, laundry, bedmaking, cleaning, etc.); prevocational/vocational skills (staying on task, accepting supervision, getting materials, etc.);outcomes per specialty services (i.e. OT,PT,ST); behavioral (i.e. self-control, motivation, interpersonal relationship, etc.); etc. Accurate documentation as relates to outcomes must be maintained.
- Personnel Care/Assistance: These tasks may include providing assistance with eating, personal hygiene, and other activities of daily living. In addition, ambulatory assistance and training in these tasks, and medical monitoring such as monitoring vital signs, monitoring medications, assist the person in taking medications, recognizing and reporting symptoms of illness and changes in health conditions, functioning in a supportive role accompanying and facilitating the participation in day services, pharmacies, medical appointments, and travel in the community; monitoring meal preparation and snacks as required by approved menus; supervise and provide assistance as needed during meal preparation, ensuring adherence to menus and diets and residents safety in the kitchen.
- Household Maintenance: Ensures the home is a sanitary/safe environment; monitors bedmaking; monitors washing; drying; folding sorting and distributing clothing; monitors vacuuming; dusting; mopping, and cleaning living areas, bathrooms, and kitchen; monitors ironing; trains residents in fire and safety precautions; develops weekly grocery list and assists with weekly grocery shopping; informs supervisor of any needed preventive maintenance of facility or needed facility maintenance and repairs.
This employee is responsible for ensuring vehicle rules are followed by all clients and/or passengers; notifying facility supervisor when van needs inspecting, regular service checks, routine or non- routine maintenance, etc.; inspecting the fire extinguisher and first aid kits monthly and notifying the facility supervisor of needed re-charging/re-stocking, ensuring the vehicle is fueled; cleaning interior and exterior of the van as needed; provides resident transportation to/from day placement, community outings, medical appointments, etc.; documents transportation pertaining to Personal Care Services.
- Supervision and/or General Welfare and Safety of Residents: This includes a knowledge of fire abs disaster procedures, emergency procedures, infection control and transportation safety; continual observation of residents health inclusive or physical, emotional, and behavioral changes and notifying appropriate supervisors per hierarchy of emergency procedures of significant changes and/or needs of residents.
- Medication Administration: Initial and annual medication administration certification is required for this employee. Medications will be administered per physicians orders and documented per medication certification procedures. This includes being knowledgeable of medication regimes, side effects, drug interactions, precautions, medication policy and medication education; monitoring of residents who self-medicate and weekly pill counts for these residents; pill counts prior to and on return of residents from therapeutic leave; may involve transportation of resident to/from pharmacy to pick up new meds., refills or pre-packaged meds for therapeutic leave; assist and monitor purchasing of over-the-counter meds; monitor for out-of-date meds; dispose of meds at the direction of the facility supervisor and per approved policies and procedures; inform facility supervisor of any med error and provide necessary information form completion of incident report following any med error; prepare adverse reaction report when allergic reactions occur; keep meds locked in medicine cabinet or locked box; consult with facility supervisor or designee for PRN med administration.
- Communication: This employee is responsible for on-going communication, both verbal and written, with residents, co-workers, supervisors. Consultants, administrative assistants, and others as needed or requested by facility supervisor.
- Miscellaneous: Participates in staff meetings and employee/supervisor conferences; participates in required in-service training; plans program schedules for evenings and special events.
REQUIREMENTS/QUALIFICATIONS:
- High School Diploma
- 1 Year of Direct Care Experience preferred but not required
- Proficient in MS Office suite, Email, Internet
- Requires strong communication skills, both verbal and written, organizational skills, both analytical and problem solving, and the ability to work with confidential documents.
- Ability to manage multiple tasks and achieve deadlines under pressure.
