Position Type: Full-Time

Salary/Rate: $22.19 Per Hour

Job Summary:

This role coordinates the daily operation of assigned Community Home(s). Together, with each person, their family, and other team members, assist in charting a course that supports each person in realizing their life of personal significance.

Minimum Requirements:

• 2 years of experience as a Direct Support Professional, with at least one year of supervisory experience or a Bachelor’s Degree.
• Ability to complete one-person transfers is required.
• Successful completion of all training as required by Achieva and government regulations, competencies, and timeframes.
• Valid PA driver's license and reliable transportation.

Job Responsibilities - Highlights:

• Collaborate with the person we support, as well as internal and external stakeholders to address specific medical and therapeutic needs.
• Provide guidance to a team of direct support professionals, ensuring a positive and supportive work environment.
• Infuse Achieva’s core values and mission as the foundation for everything you do.
• Be a liaison between people supported, their families, and other support networks to foster open communication.
• Maintain and monitor accurate records of progress, incidents, in-house and persons’ records, as well as other relevant information.
• Provide personalized care in daily living, and social activities.
• Ensure compliance with all rules, regulations, training and policies.
• Coordinate and ensure the upkeep and maintenance of the home.
• Implement plans to assist the person in achieving their personal goals.
• Develop the weekly staff schedule and ensure all shifts are covered.
• Communicate and resolve concerns.
• Participate in staff meetings.
• Assign and ensure completion of each person's Annual Assessment and supporting documents.
• Administer medications and oversee medication administration of staff.
• Must obtain First Aid/CPR certification within 6 months of start date and pass Medication administration within 2 months of start date.
• Utilize technology for all daily required tasks.

In addition to paying for federal and state clearances, Achieva offers annual training.
