JOB SUMMARY:
As a DCP (Direct Care Professional) in a Group Home, you will play a crucial role in supporting the health and well-being of individuals residing in our group home settings. Your primary responsibilities will include providing basic medical care, administering medications, and assisting in the overall healthcare management of the consumers residing in our facilities. This role requires a compassionate and patient-focused approach to address the unique needs of individuals in a group home environment. Group Home Location:
- Monroe County
- Hybrid schedules or working from home are not available options for Direct Care Staff. The employee’s physical presence in the home is a crucial service we provide to our consumers.
- Applicants must be available to work afternoons or midnights during one of the following shifts:
4pm - 12am (afternoon)
12am - 8am (midnights)
Compensation:
- $17.90/hr
Job Duties as a DCP:
Essential Tasks:
1. Personal Care Assistance:
- Assist consumers with the activities of daily living, including bathing, dressing, grooming, and toileting.
- Provide support with mobility and ensure a safe living environment.
2. Medication Management:
- Administer medications to consumers according to prescribed schedules and document the medications administered in the appropriate electronic health record.
- Document medication administration and report any concerns to supervisory staff.
3. Meal Planning and Preparation:
- Plan and prepare nutritious meals based on consumers' dietary needs, preferences, and individual consumer menus.
- Assist consumers with eating and encourage healthy nutritional habits.
4. Emotional and Social Support:
- Foster positive relationships with consumers and create a supportive and inclusive atmosphere.
- Engage consumers in social and recreational activities to promote mental and emotional well-being.
5. Individualized Care Plans:
- Implement and follow individualized care plans for each consumer.
- Regularly communicate with the care team to update and modify care plans as needed.
6. Monitoring and Reporting:
- Monitor and document consumers' behavior, health status, and any significant changes.
- Report and document observations and incidents to supervisory staff promptly.
7. Crisis Intervention:
- Respond calmly and effectively to crisis situations, following established protocols.
- Collaborate with team members to de-escalate challenging situations.
8. Maintaining a Clean and Safe Environment:
- Make certain the group home environment is clean, organized, and free from safety hazards.
- Perform light housekeeping duties and encourage consumers to participate in maintaining their living space.
9. Documentation and Record Keeping:
- Complete and maintain accurate documentation, including daily logs, incident reports, and progress notes in the appropriate electronic software system.
- Uphold confidentiality and privacy standards regarding consumers' information.
Mandatory Qualifications:
- High school diploma or equivalent.
- Relevant education and/or training is desirable.
- Previous experience in a caregiving or direct care role. If possible, in a group home or similar setting.
- Employees need to have a compassionate and patient demeanor with genuine interest in supporting individuals with diverse needs.
- Strong communication and interpersonal skills.
- Ability to work independently and/or collaboratively within a team.
- Basic knowledge of health and safety regulations. Working Conditions:
- The position may involve standing, walking, and assisting with lifting and transferring consumers.
- A flexible schedule may be required to accommodate the needs of consumers and the facility.
