We need an RN Care Coordinator to join our team at The Advanced Therapy Center at Chesterwood Village in West Chester, Ohio.

36 to $43 Per Hour
You'll collaborate with skilled residents, families, and the Interdisciplinary Team (nursing, therapy, social services, and case management) to coordinate care and services delivered during skilled stays (upon admission up to 30-days post-discharge).
You'll complete admission assessments with residents / family within 48 hours of admission in order to facilitate resource management, treatment plan formulation, and discharge planning. You'll also effectively communicate with residents, families, and the Interdisciplinary Team to promote patient-centered care and improve clinical outcomes.
You'll Be Responsible For :
Developing individualized plans of care & communicating them to the appropriate stakeholders
Identifying the social, emotional, physical, mental, & financial needs of each resident
Coordinating & communicating with nursing staff regarding clinical needs during the skilled stay
Collaborating with therapy to identify therapy goals
Communicating expected length of stay with residents / families to assist with discharge planning
Collaborating with social services to meet the resident’s psychosocial & socioeconomic needs during the skilled stay and upon discharge (including arranging for follow up community services, home health referrals, and alternative placements)
Collaborating with Case Management to complete accurate & timely documentation to support on-going skilled need
Ensuring high quality care is provided as efficiently & cost-effectively as possible during the skilled stay
Facilitating continuity of care by coordinating the transition from skilled stay to post-discharge & ensuring the resident needs are met to prevent rehospitalization
Attending daily clinical rounds, twice-weekly UR (Utilization Review) meetings, and Care Conferences to collaborate with the Interdisciplinary Team (nursing, social services, therapy, and case management) on skilled stay & post-discharge needs
Practicing professional nursing & promoting patient advocacy in accordance with the ANA (The American Nurses Association) Code of Ethics for Nurses
You'll Need To Have :
An active, unencumbered Ohio RN license (BSN preferred)
At least 5 years of skilled nursing experience in a long-term care setting
General knowledge of Medicare Part A & Managed Care insurance plans in the skilled nursing setting
General knowledge of how the Interdisciplinary Team functions individually & with each other in skilled nursing facilities (nursing, therapy, social services, and case management)
An active American Heart Association BLS / CPR certification (with in-person skills portion
The ability to work Monday-Friday 8 : 00am-4 : 30pm with an On-Site Weekend Rotation once every 5 weekends (will take off a weekday before / after the on-site weekend rotation)
