Homeless Prevention Specialist - Fond du Lac/Oshkosh
ADVOCAP’s Homeless Prevention program is accepting applications for a Homeless Prevention Specialist for our Fond du Lac or Oshkosh offices. Our Homeless Prevention program is in Fond du Lac, Green Lake, and Winnebago counties. We assist individuals and families to overcome homelessness and/or potential homelessness and poverty by establishing and implementing an individual plan for them to achieve self-sufficiency, especially housing stability.
Responsibilities Include:
- Respond to individuals and families in crisis and assist them in addressing basic needs
- Work with individuals and families in establishing their plan for self-sufficiency
- Coach participants along their path to self-sufficiency
- Connect participants to appropriate community resources and document the individual’s needs, plans, services provided, and outcomes achieved
- Identify gaps in community services
- Maintain records documenting participant progress
- Assess and enroll participants in appropriate programs
- Establish trusting relationships with people
Job Requirements:
- This position will have a bachelor’s degree in human services or social services, and 2 years of previous case management work experience or an equivalent combination is required to meet job standards.
- Possess an understanding of poverty and the ability to be non-judgmental with those served
- Ability to develop positive, growth-directed relationships with individuals and families in poverty
- Knowledge of and experience in utilizing community resources to help people in need
- Acceptable drug screen and comprehensive background check required
- Reliable attendance and a professional work ethic
Salary and Benefits:
This full-time position will work 36 hours per week with a flexible schedule between Monday — Friday. The entry/starting wage for this position is $21.50 per hour, negotiable based on experience. This full-time position will have access to health, dental, vision, short- and long-term disability, vacation, sick and holiday pay, and our employer contribution 401k.
