POSITION SUMMARY: Under the leadership of the Assistant Vice President of Health Center Operations, provides care coordination between resettlement agencies and Affinia Healthcare for the safe and efficient care and services to newly arrived refugee patients, supervises the Refugee team including but not limited to; Coordinators, Educators, & Specialists). Tracks progress towards program goals and ensures timely completion of program reports. As a member of the health care team, assist in the planning and implementation of patient-based care based upon sound nursing judgment, clinical preparation, knowledge, skills, and experience.
POSTION QUALIFICATION REQUIREMENTS
Education: Graduate of an accredited school of nursing.
Experience: Requires (3) three years' experience working in a community health setting and with refugee and immigrant patients. Experience coordinating clinic services.
Organizational and communication skills as well as ability to work under strict deadlines; ability to work as part of a team, including but not limited case workers, employment specialists, interpreters and others; experience in administration and or program supervision, team leader with crisis management experience; sensitive to cultural differences of clients and staff from diverse backgrounds; functional vehicle required; able to work flexible hours.
Serves as a liaison between external stakeholders and Affinia Healthcare staff when needed.

Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri.

FUNCTIONS OF THE JOB
Essential Functions
- Acts as the point person (primary coordinator) between resettlement agency and Affinia Healthcare to schedule medical assessment appointments, request interpreters, and address transportation needs of newly arrived refugees.
- Train and supervise refugee team members (Refugee coordinator(s), educator(s), and specialist(s), ensuring individual and group health education goals are met.
- Ensure clients receive support from the refugee team through education, coordination, interpretation and other services as needed.
- Collaborate with outside organizations and partners to provide coordinated medical services for newly arrived refugees/refugee families.
- Educates clinic staff and others regarding culturally sensitive issues related to refugees.
- Maintains accurate written and electronic records in accordance with funding and agency requirements.
- Prepares reports and ensures compliance of requirements with the United States Committee for Refugees and Immigrants (USCRI).
- Attends refugee resettlement quarterly meetings.
- In collaboration with health center Quality Improvement Coordinators (QIC), identify and implement opportunities for program improvement.
- Works with subject matter experts to ensure current and consistent practices are adopted and sustained.
- Facilitate communication among program staff regarding program specific procedures, changes, clarifications, and distribution of pertinent materials.
- Assists in the management of refugee patient care between visits.
- Performs specialized procedures, examinations and/or tests as ordered.
- In collaboration with physician, provides extensive health education, counseling and advice to patient and families on topics such as lifestyle changes, diabetic controls, etc., effectively assesses patient educational needs, prioritizes patient's learning needs, identifies barriers to learning, develops teaching plan, and uses various educational tools to activate the non-compliant patient.
- Assists Assistant Vice President and Clinic Manager with patient flow. Acts as patient advocate to ensure needs are met.
- Obtains and documents reason for visit, patient vital signs, including temperature, blood pressure, weight, respiration, pulse, and BMI.
- Following defined protocol independently assesses patient/callers' health status based upon presenting symptoms. Determines the urgency for care and directs patient/caller to most appropriate level of care, or an acceptable alternative venue for care of patients with acute problems should access not be readily available at the clinic. Seeks provider advice appropriately.
- Use computer knowledge to assess patient information, follow-up appointments, e-mail, etc.
- Performs other duties as assigned.
Marginal Functions: Monitors completion of medical records for clinic session and patient confidentiality.
Supervises: General supervision of refugee team (coordinator, educator, specialist).
