Primary function is to facilitate the referral intake process, coordinate care with the
interdisciplinary team, the patient/client,
their family, and the referral source
Job Qualifications:
Education: Graduate of an accredited school of Vocational/Practical nursing.
Li-censure: Current state license as a Licensed Vocational Nurse or a multi-state license issued by a state member of the National Nurse L:censure Compact (NLC). Current drivers license.
Experience: One year experience as a Licensed Vocational Nurse/Licensed Practical Nurse, home health experience preferred.
Skills: Nursing skills as defined as generally accepted standards of practice. Good interpersonal skills.
Transportation: Reliable transportation. Valid and current auto liability insurance.
Environmental and Working conditions:
Works in a routine office environment; noise level may be moderately high; must have the ability
to work a flexible schedule and extended hours. Possible exposure to toxic materials, toner, etc.
Physical and Mental Effort:
Requires working under some stressful conditions to meet deadlines and patient/client needs, and to
make quick decisions. Requires excellent problem solving skills.
