GENERAL PURPOSE:
Under the supervision of the Outreach Program Manager, the Outreach and Enrollment Specialist provides informal community based health related services and establishes vital links between health providers and persons in the community throughout the Ampla Health service areas.
MAIN RESPONSIBILITIES & DUTIES:
- Assists patients calling or visiting the clinic
- Performs patient education, translation and arrangements for patient transportation.
- Counsels and informs the patient population about the medical and dental services and all other programs available to them through Ampla Health.
- Assists in the scheduling of patients for services at clinics throughout the area.
- Brings patient concerns to the attention of the Patient Advocate and Site Administrator.
- Maintains the confidentiality of patient records in accordance with HIPPA regulations.
- Process applications for eligibility of Medi-Cal, CMSP, and programs.
- Acts as a liaison between Ampla Health and various community organizations.
- Keeps the Outreach Program Manager informed of local community events and reports weekly.
- Attends in-service classes, institutes and seminars to update knowledge and skills in areas of interest and need as directed by the Patient Navigator.
- Attend classes in linguistics and cultural competency.
- Assists community members with the completion of application/enrollment forms for various programs of health coverage for which they may qualify, including FPact, CHDP, Healthy Families, Sliding Fee, etc.
- Other duties as assigned by supervisor.

PROFESSIONAL KNOWLEDGE, SKILLS & ABILITIES:
- High school graduate or equivalent
- Current CPR certificate
- At least two years experience in community, social services activities preferred
- Ability to travel to other locations as needed
- Valid California Driver’s License and proof of insurance
- Reliable method of transportation for required travel
- Excellent communication skills
- Bilingual (English/Spanish) preferred
- Ability to relate to and work with people of all ages, social, cultural and ethnic backgrounds
- Demonstrate clear knowledge of Ampla Health’s clinic structure, standards, procedures and protocols
WORKING CONDITIONS & PHYSICAL REQUIREMENT:
Works well with patients in a generally comfortable environment office. Employees must possess the following physical requirements:
1) Must be in good health and able to lift up to 40 lbs. and push up to 100 lbs. (on wheels)
2) Must be able to hear staff on the telephone and those who are served in person, and speak clearly in order to communicate information to clients and staff
3) Must be able to read memos, forms, and other pertinent written materials
4) Ability to operate standard office equipment such as a computer, telephone, fax machine, copier, etc.
