What are the responsibilities and job description for the Targeted Case Manager position at Astra Behavioral Health?
The Targeted Case Manager position is responsible for managing an assigned caseload of clients having severe and persistent mental illness or severe emotional disturbance and assessing client needs, developing, implementing and reviewing service plans, and working with other community resources in meeting/achieving client service needs.
ESSENTIAL FUNCTIONS/MAJOR RESPONSIBILITIES:
1. Participates in screening of new service requests within the clinic and outside providers by means of agency approved procedure, which ensures that all individuals are adequately and appropriately served according to the individual’s diagnostic criteria and needs.
2. Accepts newly assigned cases in which the diagnostic intake has been completed by a licensed provider. Completes case management assessments on all clients entering caseload.
3. Develops appropriate treatment service plans with clients as the means for implementing appropriate services and updates according to regulations.
4. Provides on-going supportive and/or case management functions in accordance with the problems, needs, and strategies identified within the treatment/service plan in order to help the clients to achieve the stated goals and objectives.
5. Provides face-to-face reviews with the client and/or relevant staff on a regular basis the progress made in reaching service goals so that the treatment/service plan can be modified as necessary to ensure that the goals and objectives are being achieved. The frequency of reviews will be determined by relevant need or requirements but not to exceed more than every three months.
6. Services to be provided a minimum of two face-to-face contact for SMI, SED, and SUD and a minimum of three face-to-face for CCC. Additional contacts of a minimum of two will be provided through collateral and networking.
7. Records will be maintained in accordance with agency and regulatory standard and requirements.
8. Meets regularly with the immediate supervisor as a means of enhancing professional growth, reviewing and processing the provision of targeted case management services, and dealing with appropriate administrative issues.
9. Maintains close communication with consulting medical and counseling providers for input regarding medication compliance, side effects of medication, and medication or treatment changes and alerts these providers of any changes in client adjustment which might suggest decompensation and a need for more aggressive intervention.
10. Performs other duties as assigned by the supervisor which are consistent with the position and in compliance with agency policies and procedures.
QUALIFICATIONS:
- Must possess a combination of mental health work experience or relevant education and training, which indicates the possession of knowledge, skills, and abilities necessary to perform essential job duties.
- A minimum of a Bachelor’s degree and one-year experience post-graduation in an accepted field for SMI, SED, and SUD. Also, a minimum of a Bachelor's degree and a minimum of five years post- graduation experience in an accepted field for CCC.
