Overview: We are seeking a dedicated and compassionate Targeted Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals need.
Duties:
- Conduct assessments to determine client needs and develop individualized care plans
- Provide mentoring and support to clients, helping them navigate through challenges - Coordinate and monitor services, ensuring clients receive necessary resources and referrals
- Collaborate with other professionals, such as childcare providers, behavioral health specialists, and addiction counselors, to ensure comprehensive care
- Assist clients in accessing community resources, such as Section 8 housing, Medicare benefits, and workforce development programs
- Provide crisis management intervention when necessary
- Advocate for clients' rights and ensure their voices are heard in decision-making processes
- Maintain accurate and up-to-date documentation of client interactions and progress
Experience: - Bachelor's degree in social work or related field required, Masters Degree Preferred.
Previous experience in Targeted case management or a related role is highly desired
Familiarity with behavioral health issues and addiction counseling
Strong organizational skills with the ability to prioritize tasks effectively
Excellent communication and interpersonal skills
Ability to work independently as well as part of a team
Proficiency in Microsoft Office Suite

Pay: $20.00 - $26.00 per hour
