What are the responsibilities and job description for the Targeted Case Manager position at Bluegrass Recovery Center?
Responsibilities:
- Assist clients in gaining access to needed medical, social, educational or other service supports
-Assessments of clients medical, social and functional status
- Preparation and maintenance of case record documentation to include care plans, forms, reports, etc.
- Develop and prepare individual care plans based on assessment findings
-Collaborate with clients and families to establish and maintain working relationships --Facilitate referrals and communication between service providers
-Participate in regular Treatment Team Meetings
-Ensure the company adheres to state and federal licensure and regulations
-Maintain client records
Qualifications:
-Certified in Targeted Case Management in Kentucky
-Bachelor's degree of arts or science or Certified alcohol and drug counselor
-Have 1 year working directly with adults in the human service field -Knowledge of local community resources, treatment options and support services -Strong understanding of Kentucky targeted case management regulations, policies and procedures related to substance abuse disorders
-Strong communication and interpersonal skills
-Ability to work independently and as part of a multidisciplinary team

Pay: $20.00 - $24.00 per hour
Expected hours: 40 per week
