Examples of Duties and Responsibilities:

Redetermination Lists
-
Crisis/Residential Logs- keeping track of hospitalizations and discharges

Travel to Hospitals weekly/as needed for referrals and follow ups (Catchment Areas and CSU/Lakehaven)

Responsible for coordinating care and providing guidance to primarily low-risk patients.

Covering caseloads on FMLA, expected contacts as needed.

Direct/indirect care with individuals

Provide case management services, refer, and facilitate access to community services and supports.

Conduct training, outreach, and marketing

Respond to crisis situations compassionately and appropriately.
Minimum Qualifications:
Must have a minimum of two years experience as a case manager and graduation from an accredited college or university with a minimum of a Bachelor’s degree in a health related field of counseling, psychology or social work, or must be licensed as a nurse (LPN or RN). LPN’s must have fifteen semester hours of behavioral health and one year of behavioral health care experience. Individuals with a Bachelor’s degree in rehabilitation, occupational therapy, criminal justice, and education must have fifteen college-level semester hours of course work in behavioral health and an additional one year of work experience in the behavioral health setting.
