Collaborating with an extensive number of funders, volunteers, and supporters, Catholic Community Services of Lane County (CCSLC) is a 70-year-old 501(c)(3) social services agency dispensing the largest volume of no-cost basic assistance- food, utilities, clothing- to individuals and families living in poverty throughout Lane County. We foster a “whatever it takes” approach with the case management and housing navigation services we provide to our unhoused neighbors. As well, our Refugee & Immigrant Services Program is rapidly growing and is the only Mid-Willamette Valley team offering a coordinated effort of legal assistance, wrap around supportive services and employment connections to refugees and immigrants who arrive in our community. POSITION TITLE: Case Manager I
SUMMARY:
Join our Supportive Services Team! Our Case Manager I supports families by collaborating with them to develop Service Plans intended to increase self-sufficiency and identify opportunities. This is accomplished through home visits, identifying risk factors, and connecting families to community resources such as employment development, educational opportunities, healthcare, and housing resources. The Case Manager will also track family development and progress, formulate discharge plans, and coordinate with other providers to support the Service Plan goals. REPORTS TO: Associate Director, Supportive Services
WORK LOCATION: Springfield, OR-1025 G St., and in the community
FLSA STATUS/ /HOURS: Non-Exempt, Full-time, 40 hrs./wk. M-F, evenings or weekends if needed, special events
COMPENSATION: $19/Hr.
QUALIFICATIONS, KNOWLEDGE, SKILLS & ABILITIES
- Associate’s degree in Human Services field or higher, or two years related work experience
- Bilingual English/Spanish preferred
- Two years experience accessing community services, housing resources and benefits
- Ability to assess and develop individualized case plans
- Empathy & compassion for those in need and for diverse populations
- Ability to work independently and in a team environment
- Ability to direct and train interns and volunteers, as needed
- Valid Oregon Driver’s License, reliable transportation, automobile insurance, and ability to drive for work purposes.
