The Service Coordinator serves residents of HUD subsidized housing who are 62 years or older to help them live independently and increase their quality of life. They assess and connect residents with community-based services that help them Access Cost Saving Benefits (e.g., Medicaid, SNAP, Food Pantries), Maintain Health and Wellness (Monitor IADLs and ADLs), Increase Support System (Skilled Nursing, Family, Decrease Isolation), Maintain Housing (Homemaker, Lease Education). Services are provided through individual and group interventions. In addition, the Service Coordinator collaborates with the property management team to support residents’ success with the lease and house rules and avoid eviction.

Key Responsibilities
- Conduct intake and assessments to identify service needs of individuals and the community more broadly
- Link residents to services and resources
- Monitor service provision and follow up with residents
- Coordinate educational and wellness programs
- Report suspicion or incidents of abuse, neglect, or exploitation
- Partner with property management team to help residents maintain their housing
- Meet expectations for data collection and annual HUD report
- 37.5 Hours/week TBD: Works within Monday – Friday, business hours; occasional evenings; no on-call responsibilities.
- Associates in Human Services or Community Health; Bachelor’s Degree in Social Work, Gerontology, Psychology, Counseling or Public Health preferred.
- Current driver’s license.
- One (1) plus years’ experience with case management, advocacy and problem solving.
- One (1) plus years’ serving seniors preferred.
- Experience working independently.
- Experience working with HUD residential locations a plus.
- Exceptional customer service delivery to maintain effective relationships.
- Demonstrated experience with Microsoft office software.
- Pass Criminal background check and drug-screening.
