What are the responsibilities and job description for the Transition Care Management RN position at CentraState Healthcare System?
Overview
Transition Care Management RN ensures the recently discharged patient a smooth transition from the hospital back to their home environment. The TCM nurse assures that the patient’s post-discharge plan of care, education, and social needs are addressed to prevent unnecessary emergency department visits or inpatient readmissions.
Responsibilities
In accordance with the “Well Prepared” grant from the Grunin Foundation, the TCM Nurse acts as a consultant to the hospital throughout the grant-period during the testing phase of the grant. Advises the hospital regarding opportunities for success and whether and how to sustain and scale the project at the end of the grant period.
Qualifications
- Registered Nurse with valid New Jersey License
- Strong clinical judgement and ability to assess patient needs
- Excellent communication and Interpersonal skills to effectively education patients and caregivers
- Knowledge of disease management and medication adherence strategies
- Proficiency in electronic health records and technology assisted condition management
