Overview

Transition Care Management RN ensures the recently discharged patient a smooth transition from the hospital back to their home environment. The TCM nurse assures that the patient’s post-discharge plan of care, education, and social needs are addressed to prevent unnecessary emergency department visits or inpatient readmissions.

Responsibilities

In accordance with the “Well Prepared” grant from the Grunin Foundation, the TCM Nurse acts as a consultant to the hospital throughout the grant-period during the testing phase of the grant. Advises the hospital regarding opportunities for success and whether and how to sustain and scale the project at the end of the grant period.

  • Performs outreach to calls exclusively to eligible candidates for the grant funded “Well Prepared” home-based hospital readmission prevention program. Enrolls willing patients for 30-day post discharge follow up for patients recently discharged from the ED or Inpatient setting as outlined by the grant proposal.
  • Initiates phone calls or home visits to check on patient progress, address concerns and provide support.
  • Assess patient needs and risk factors for readmission based on medical history, medication regimen, health related social needs, and functional abilities.
  • Develops individualized transition plans in partnership with the patient that includes specific care goals, follow up appointments, medication instructions, chronic condition education, and social care assistance as needed.
  • Delivers patient and caregiver education by providing clear and concise education to patients regarding their medical condition, medication management, symptom monitoring and self-care practices to promote adherence to treatment plans. Utilizes the teach back method.
  • Connects patients with appropriate community resources such as home healthcare services, diabetes self-management education programs, transportation assistance, food assistance, and other community-based support programs.
  • Collaborates with the healthcare team including physicians, social workers, case managers and other healthcare professionals to coordinate a seamless transition to home.
  • Documents significant patient interactions and tracks key metrics related to enrollment, education encounters, patient readmission rates, patient satisfaction, and resolution of social needs. Evaluates effectiveness of interventions upon case closure. Prepares final report at the conclusion of the grant funded program.

    Qualifications

    • Registered Nurse with valid New Jersey License
    • Strong clinical judgement and ability to assess patient needs
    • Excellent communication and Interpersonal skills to effectively education patients and caregivers
    • Knowledge of disease management and medication adherence strategies
    • Proficiency in electronic health records and technology assisted condition management
