JOB SUMMARY:
The LVN Nurse Supervisor is responsible for patient care needs through individual and Team use of inbound and outbound telephone protocols. The LVN Nurse Supervisor continuously demonstrates positive customer relationship skills by assessing caller needs through active listening, clinical assessment, education and advanced problem-solving abilities. The LVN Nurse Supervisor will actively manage patients’ immediate and ongoing needs with attention to ensuring positive patient outcomes.
DUTIES AND RESPONSIBILITIES:
- Demonstrates superior critical thinking and problem-solving skills. Demonstrates thorough knowledge and use of the nursing process utilizing age appropriate knowledge and assessment skills by identifying physical, psychosocial and development needs and adapting interventions accordingly. Cultivates an atmosphere of patient-focused care.
- Demonstrates excellent written, verbal, telephone and interpersonal communication skills. Utilizes excellent hearing and listening skills in order to receive and act on detailed information. Communicates based on the client’s level of comprehension and understanding. Demonstrates diplomacy and sound judgment in all interactions; respect for multicultural diversity.
- Demonstrates excellent organization and multitasking skills. Must be able to apply critical thinking, communication and documentation skills simultaneously.
- Demonstrates ability to manage immediate and ongoing client needs with attention to detail and use of appropriate interventions.
- Demonstrates knowledge of community resources.
- Utilizes computer programs to assess, process, educate and document all information in an accurate manner. Demonstrates working knowledge of all utilized software programs.
- Meets departmental quality and productivity standards.
- Participates in departmental Quality improvements and initiatives.
- Participates in continuous improvement activities; provides constructive feedback for continued growth of the team.
- Participates in staff meetings, departmental in-services, patient care conferences, committees, projects and other continuing education activities.
- Evaluates environmental safety and maintains an environment that is safe.
- Delegates and assigns duties within the assignee’s scope of practice and /or competency; and consistent with the assignee’s job description and CentroMed’s policies.
- Other duties as assigned
QUALIFICATIONS:
- Education: Graduate of an accredited Vocational Nurse educational program. Must be currently licensed with the Board of Vocational Nurse Examiners for the State of Texas. CPR Certification.
- Experience: Supervisory experience in a Family Medicine setting, highly preferred Knowledge of outpatient triage, prescription refills and Medical Home practices preferred. Knowledge of community resources, Telehealth experience, Bilingual - Spanish
- Other: Operational knowledge of physician practice. Knowledge of health insurance and managed care.

PHYSICAL REQUIREMENTS:
- Must be able to perform the essential functions of the position.
