Overview
The Community Resource Specialist position works to meet the tangible needs of patients and families. Responsibilities include sharing of pertinent patient information to the team, performing outreach and education to patients and families, assisting with tasks, and helping identify opportunities to enhance care coordination in collaboration with internal and external team members.
This position is grant-funded for an initial period of one year, with the potential for a multi-year extension depending on future funding.

Salary : $22
