Overview
The Lead Community Resource Specialist position works to meet the tangible needs of patients and families. Responsibilities include sharing of pertinent patient information to the team, performing outreach and education to patients and families, assisting with tasks, and helping identify opportunities to enhance care coordination in collaboration with internal and external team members. This role also helps support and provide guidance to the CFCC Community Resource Specialist Team.
