Director, Case Management and Family Experience
We are expanding and are hiring a Director, Case Management and Family Experience. This is a hybrid role; role is responsible for travel throughout NJ.
In this role, you will shape an industry-leading case management program for children diagnosed with Autism and their families. You will work closely with the Regional Clinical Director to ensure case management is fully synced with the ABA clinical program to provide seamless support in onboarding, social determinants of health, care navigation across providers, case management, and effective transition from ABA at graduation.
You will hire, supervise, coach, and mentor a team of leaders within case management and care coordination. And you will be part of the state clinical leadership team responsible for overall experience and outcomes of a population that will grow to 1000 kiddos.
The ideal candidate combines social work experience with knowledge of Autism care journeys and exposure to multiple sides of the Autism provider landscape.
Key Responsibilities :
Case Management Vision :
- Set vision for what a best-in-class family journey should look like from onboarding to post-graduation support; define the case management / social work support needed to realize the vision.
- Align stakeholders around program requirements including capabilities, staffing ratios, activities, and cross-functional / cross-organizational coordination
- Detail the plan to scale.
