Job Summary
We are seeking a dedicated and compassionate Ambulatory Case Manager to join our team. The Case Manager will play a crucial role in coordinating patient care, ensuring that individuals receive the necessary services and support throughout their treatment journey. This position requires strong communication skills, a thorough understanding of case management processes, and the ability to work collaboratively with patients, families, and healthcare providers in a fast-paced environment.
Duties
- Assess patient needs and develop individualized care plans that align with treatment goals.
- Coordinate care across various departments.
- Facilitate discharge planning to ensure smooth transitions from acute care settings to home or other facilities.
- Collaborate with healthcare providers to optimize patient outcomes.
- Monitor patient progress and adjust care plans as necessary based on ongoing assessments.
- Provide education and resources to patients and families regarding managed care options and available support services.
- Advocate for patients’ needs within the healthcare system to ensure they receive appropriate services.
- Maintain accurate documentation of case management activities in compliance with regulatory standards.
Requirements
- Proven experience in case management within a clinical or acute care setting.
- Familiarity with managed care principles and discharge planning processes.
- Strong knowledge of healthcare systems, including emergency medicine protocols.
- Excellent communication and interpersonal skills for effective collaboration with diverse populations.
- Ability to work independently as well as part of a team.
- Proficiency in using electronic health records (EHR) systems for documentation purposes.
- Certification in case management (CCM) is preferred but not required.

Pay: $26.00 - $32.50 per hour
