Onondaga Case Management Services, Inc. dba Circare is a not-for-profit community-based mental health agency located in Syracuse, New York. Circare programs are designed to help individuals and families who have encountered certain obstacles develop the resources and supports to live a satisfying, naturally independent life.
The Coordinator of Rehabilitation Services will provide administrative support for direct care providers who serve clients in our Community Oriented Recovery & Empowerment Services (CORE), Home & Community Based Services (HCBS) and ACCES-VR Programs. It is expected that the Coordinator will also provide direct care services to a small number of clients.
This is a Full-Time position working a flexible schedule designed to meet the needs of the Programs.
Principle Duties and Responsibilities:
· Supervise a team of Peer Specialists and Employment Specialists
· Provide at least 1 type of CORE, HCBS or ACCES-VR service:
o At minimum, be able to qualify to provide CORE Psychosocial Rehabilitation services, which requires a HS/GED diploma plus online Center for Practice Innovations (CPI) training
o Qualifying for CORE Peer services, HCBS or ACCES-VR services is a plus
· Coordinate Clinical Supervision of the Specialists with the assigned Circare licensed staff
· Full knowledge of CORE, HCBS, and ACCES-VR Programs
· Supervisor-level working knowledge of Circare’s technical products, including client databases, communication vehicles, learning systems, and common software.
· Manage Program Intake processes for new referrals and assign them to Specialists.
o Coordinate the process with the LPHA to ensure medical necessity requirements are met
· Ensure delivery of services & all documentation adhere with Program requirements, Circare’s Compliance Program and the agency’s Policies & Procedures.
· Manage Program Discharge processes to ensure compliance standards are met.
· Conduct routine internal Program auditing and monitoring.
· Assist Circare colleagues and auditors with external audits, including gathering & organizing client documentation
· Interview, hire, and train new Program staff.
· Verify work-required credentials for all candidates and monitor staff credential renewals.
· Ensure that all necessary staff training is completed at Orientation and annually thereafter.
· Anticipate and manage Program needs for increased referrals, including monitoring & evaluating Specialist caseloads, provide outreach to Circare Care Managers and to other targeted referring organizations, and maintain up-to-date Program promotional materials.
· Present Program updates to agency leadership and other staff as requested.
· Keep up-to-date on professional development and certifications.
· Other tasks as assigned.
Job Requirements:
· Must have a BA and 2 years of supervisory experience or a Master's Degree and 1 year of supervisory experience.
· Must have at least 3 years’ experience working as an employment counselor
· : 3 years’ experience working as an education counselor
· Must have at least 2 years’ experience working in mental or behavioral health
· Must pass the suitability background checks for each Program
· Must have good computer skills, knowledge of Microsoft 365 Office Suite & Microsoft Teams
· Must have good written and communication skills
· : have a Community Work Incentives Partner (CWIC) or Community Work Incentives Practitioner (CWIP) from Virginia Commonwealth University or Cornell University.
· Must deliver client services in the community
· Must have a their own personal vehicle, a valid driver’s license, and sufficient auto insurance:
o Circare’s insurance carrier requires that Circare employees who drive personal vehicles for work maintain minimum auto insurance coverage limits of at least $100,000/person; $300,000/accident and that employees provide HR with proof of current minimum insurance coverage (copy of Auto Liability Insurance Declaration Page) on their first day of employment. Employees are required to provide an updated Auto Liability Insurance Declaration Page to HR by expiration date on Declaration Page.
