Overview
We are seeking a dedicated Social Worker to join and lead our BRAND NEW team. The ideal candidate will be passionate about helping seniors/geriatric patients to remain in the comfort of their homes.
Duties
1. Conducts an initial Social Work assessment and scheduled and as-needed reassessments for all participants.
2. May participate in completion of Determination of Need (DON) document as part of enrollment eligibility assessment.
3. Performs all functions and duties as required as a member of the Interdisciplinary Team (IDT), including, but not limited to, participation in IDT meetings, family conferences, grievances, service determination requests, CMS audits.
4. Coordinates with IDT in developing individualized plans of care for each participant.
5. Performs comprehensive psychosocial assessments in accordance with practice act: for the evaluation, treatment, and prevention of mental and emotional disorders in individuals, families, and groups based on knowledge and theory of professionally accepted theoretical structures, including, but not limited to, psychosocial development, behavior, psychopathology, unconscious motivation, interpersonal relationships, and environmental stress
6. Establishes and maintains a working rapport with participants.
7. Maintains and promotes patient satisfaction through acknowledgement that environmental, social, and cultural factors influence one’s health.
8. Assures completion of assessments in appropriate time frame
9. Identifies warning signs of mental health distress, communicates findings with IDT, makes recommendations to behavioral health counselors or other professionals as needed.
Required
MUST have a MSW
LSW or LCSW a major plus
Experience with Medical Social Work or working with elder adults
Job Type: Full-time
Pay: $62,672.00 - $75,000.00 per year
