Skilled Nursing Visits - RN Case Manager

Responsibilities

A healthcare provider specializing in home and community-based services is seeking a Clinician to deliver skilled nursing care to pediatric and adult patients. The role involves managing patient care plans, ensuring compliance with regulations, and collaborating with interdisciplinary teams. Key responsibilities include:

• Managing a weekly caseload and providing care per physician orders.
• Documenting patient assessments and communicating with families.
• Administering treatments and educating patients/caregivers.
• Participating in clinical meetings and quality improvement efforts.
