Care Management Coordinator

About The Role

As a Care Management Coordinator in our Health Care Services team, you will play a crucial role in ensuring the smooth and efficient delivery of care to our patients. You will be responsible for coordinating and managing patient care plans, collaborating with healthcare providers, and ensuring that patients receive the appropriate level of care. Your work will directly impact the quality of care our patients receive and contribute to their overall health and well-being. This is a part time position with the benefit of having flexible hours.

Minimum Qualifications

• Associate's degree in nursing or related field
• Current and unrestricted RN license
• 2 years of experience in care management or related field preferred but not required
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
  
  

Preferred Qualifications

• Experience working with electronic health records and digital platforms
  
  

Responsibilities

• Coordinate and manage patient care plans, monitor for timely follow up from hospitalizations and ER visits, and ensuring that patients receive the appropriate level of care
• Collaborate with healthcare providers to ensure that patients receive timely and effective care
• Monitor patient progress and adjust care plans as necessary
• Maintain accurate and up-to-date patient records
• Provide education and support to patients and their families
