Referral Coordinator

The Community Health Center of Franklin County is a Federally Qualified Health Center dedicated to providing comprehensive, affordable healthcare services to our community. We are seeking a highly organized and detail-oriented Referral Coordinator to join our team.

The Referral Coordinator is responsible for coordinating patient referrals to specialists and other healthcare providers. This role involves managing referrals, obtaining authorizations, and ensuring a seamless transition of care for patients.

• Process patient referrals to specialists and other healthcare providers.
• Obtain necessary authorizations from insurance companies.
• Coordinate appointments and communicate with patients and providers.
• Follow up on referrals to ensure timely appointments.
• Maintain accurate referral records and documentation.
• Collaborate with the healthcare team to ensure efficient patient care.

• High school diploma or equivalent required.
• Previous experience in healthcare administration or medical office preferred.
• Strong organizational and time management skills.
• Excellent communication and interpersonal skills.
• Proficiency in computer systems and software.
• Ability to multitask and prioritize tasks effectively.
