Mobile Crisis Case Manager
GENERAL RESPONSIBILITIES:
- Respond to persons in crisis in the community. Depending on the situation, response will include a therapist who may also be in-person, on via telephone or telehealth.
- Live/in-person follow up with clients post crisis (next day and possibly for several days).
- Live/in-person family check in to provide support post crisis.
- Ensure people make it to post crisis appointments – drive them, otherwise accompany them to appointments.
- Co-respond with law enforcement when requested.
- If individual in crisis is at emergency room (ER), set with them at ER so law enforcement can return to regular duties until therapist arrives.
- Live/in-person post hospital discharge or diversion follow up.
- If multiple crises at once (which does happen in Region I and II) attend to person in crisis waiting to be seen, to provide support and learn more about what is happening (listening and being present does wonders).
- Outreach/relationship building with law enforcement to help us achieve goals regarding this position.
MINIMUM QUALIFICATIONS:
Bachelor's degree in social work/mental health/human services/education or 4 years of work experience in social work/mental health/human services/education and/or a combination of education and work experience in these fields. The Mobile Crisis Case Manager must be Peer Support lived experience qualifications as well. The individual in this position must possess demonstrated interpersonal skills, ability to work with SED children/adolescents/families and SMI/SPMI adults and families. Must be 21 years of age and have a current and valid Kansas driver’s license. Employee must successfully complete Case Management Certification as stipulated by the State of Kansas.
