This position is responsible for providing direct service to consumers with mental health issues to include crisis care, goal directed behavioral interventions, and direct support of consumers. This is a KPERS covered position.
Responsibilities:
- Provide Mobile Crisis Intervention to individuals in crisis across 4 counties as a 2 person team approach.
- Provide case management, complete assessments and treatment plans on Assertive community Treatment Team (ACT) and monitor progress of ACT clients.
- Provide proactive outreach to prevent mental health crisis and/or criminal justice recidivism.
- Collaborate with community partners (LE, hospitals, family members) to advocate for client needs during crisis situations and assist with follow up appointments as needed.
- Provide support, education, and connection to resources as needed for appropriate crisis response.
- Facilitate travel/transportation to conduct consumer crisis visits in the home or in the community as part of the Mobile Crisis Team.
- Assist with crisis visits in the office during Open Access hours and follow up on missed post hospitalization and other crisis appointments.
- Participate in agency training and ongoing professional development as required.
- Accurately complete all required documentation within expected time frame.
- Participate in required meetings for ACT, Crisis, or as assigned.
- Other duties as assigned.
Required Qualifications
- Clean KBI, SRS Adult & Child Abuse, MVR Checks
- 21 years or older
- Complete Case Management Training within 30 days of start date
- Bachelor's degree or equivalent work experience
- Valid Drivers License with favorable driving record

Job Type: Full-time
Pay: $23.56 - $25.96 per hour
