Job Overview

As an Outpatient Nurse, you will play a crucial role within our person-centered integrated care team, delivering specialized office-based services to primary health care patients while addressing their comprehensive physical and behavioral health needs.

Key Responsibilities

• Prepare patients for examinations and treatments, taking thorough histories and vital signs, and documenting findings in patient charts.
• Equip examination and treatment rooms with necessary instruments and supplies, ensuring readiness for procedures.
• Conduct venipunctures, urinalysis, EKGs, and other specified procedures.
• Administer immunizations and other injections as directed.
• Provide patient education and guidance on health management.
• Collaborate with physicians and nurse practitioners in preparing for minor surgeries and physical examinations.
• Maintain accurate patient files and records, adhering to necessary regulations.
• Monitor medical supply levels and facilitate ordering when necessary.
• Engage with patients post-discharge to review health plans and coordinate follow-up care as needed.
• Uphold a commitment to cultural competence in delivering healthcare services.
• Perform additional duties as assigned to support clinic operations.

Qualifications

• High School diploma or GED is required.
• Practical Nursing certificate (LPN) is necessary.
• A minimum of two years of experience as a licensed practical nurse in a primary health care environment is preferred.
