Pierce County’s first Certified Community Behavioral Health Clinic: Comprehensive Life Resources is looking for a qualified Case Manager to join our Outreach team in downtown Tacoma, Washington.

Work Expectations:
This is an outreach position. Meaning more than half of your work will be in the field, at homeless encampments, shelters, and/or in a community house placement. You should be aware that these conditions are often not sanitary. Being a licensed and insured driver is a requirement of the position and sometimes, employees will be expected to transport clients in their personal vehicles.
Working conditions include walking, standing, hiking, bending, cleaning, or walking upstairs for extended periods of times. It is important for your own safety that you understand you will be expected to maneuver around unsteady terrain.
Our Homeless Services are often called to intervene during all weather emergencies including the heat, snow, rain, smoke, and other extreme weather.
Responsibilities:
- Provide outreach and case management direct services to all referrals and assigned clients.
- Assume responsibility for assigned consumers, ensuring follow-up on referrals for necessary services and facilitate continuity of care.
- Become well-versed in understanding the Home and Community process, Adult Protective Services Referral, COPES, and medical systems for geriatric and disabled populations.
- Become well-versed in the behavioral health referral process, and have the ability to advocate for mental health outpatient care and Substance Use Disorder Care.
- Provide services designed to enhance quality of life and safety in the home and in the community.
- Participate in intake assessments and treatment planning for consumers. Write treatment plans in coordination with your team.
- Responsible for discharging consumers in a timely manner in both AVATAR and PDAMs.
- Provide an inventory of necessary resources and working agreements with local housing, mental health, physical healthcare, and other community providers for increased access to resources for assigned consumers.
- Provide education to facilities, professionals, and other caregivers.
- Ensure that ethical and professional standards of practice are maintained.
- Maintain clinical records and provide reports in accordance with policies and procedures.
- Participate in meetings and activities as required. Coordinate work with other staff.
- Demonstrate ongoing commitment to increasing competence and knowledge in the mental health field.
- In addition to the above, perform other duties as assigned.
Qualifications:
- Bachelor’s degree in social or behavioral sciences preferred. One-year relevant experience in mental health related field.
- Experience in the provision of case management services that demonstrate the ability to work with mentally ill population, homeless, and senior populations.
- WA State credential/license.
- Skilled in MS Word and Outlook.
- Strong analytical, interpersonal, written, and verbal communications skills.
- Demonstrated ability to multi-task, prioritize deadlines, work independently, take initiative, and maintain confidentiality.
- Valid Washington State driver’s license; proof of insurance. Ability to use one’s own car to visit consumers in the community when no company car is available.
