Description

Field Care Delivery Supervisors are practicing leaders who supervise clinical field staff while performing the responsibilities of a field nurse.

Key Details

  • Location: Primarily field-based (75% ), with the capability to perform virtual responsibilities from home.
  • Reports to: Clinical Nurse Manager
  • Direct Reports: Market field staff, who are primarily licensed practical nurses (LPNs), with potential inclusion of registered nurses, medical assistants, or paramedics depending on geographic staffing needs and constraints. Direct reports are geographically dispersed across the market, necessitating virtual supervision and travel for in-person engagement.
  • Clinical work: Responsibilities of a field nurse, including conducting home visits under the clinical supervision of a provider.

Supervisory

  • Manage field staff, including hiring, onboarding, performance evaluations, retention strategies, and ongoing support and professional development.
  • Monitor and evaluate field staff performance metrics, building their capacity to achieve objectives and goals.
  • Coordinate daily coverage and workload allocation in collaboration with market leadership.
  • Assist with clinical competency evaluations for new hires and annual reviews.
  • Serve as a clinical resource to clinical field staff, leading by example and providing ongoing guidance and hands-on support aligned with organizational and practice standards.
  • Advocate for clinical field staff by identifying gaps and barriers, proactively communicating needs, and collaborating with nursing leadership to develop and implement solutions.
  • Support medication inventory management in partnership with market operational leaders.
  • Contribute to the implementation of quality improvement programs and clinical, helping guide clinical field staff to achieve performance goals and improve care delivery.
  • Foster clinical excellence by staying current with applicable practice standards and evidence-based care and engaging staff in ongoing education and professional development.
  • Promote a practice environment that reduces work-related health risks and hazards.
  • Advance team-based care by promoting effective collaboration between field staff and providers and ensuring their integration into the interdisciplinary care team.
  • Communicates and provides direction aligned with the organization’s philosophy and objectives.
  • Champion a culture of excellence by upholding clinical and organization practice standards, advocating for improvements in care delivery, fostering clinical field staff engagement, and promoting a safe and inclusive workplace.
  • Perform other duties as assigned.

Field Care Delivery

  • Provide nursing care to patients through routine and urgent in-home visits under the direction of an authorized health care practitioner.
  • Facilitate virtual visits in the patient's home to support care delivery under the direction of a remote clinical practitioner.
  • Perform clinical procedures, including blood draws, laboratory specimen collection, medication administration, foley catheters, wound care, and removal of sutures/staples. All clinical skills require competency evaluation and are performed under the supervision of a clinical practitioner, with scope of work and level of supervision as determined by state regulations.
  • Coordinate and support patient care by implementing care plans and performing outbound calls and tasks at the direction of and under the clinical supervision of an authorized health care practitioner.
  • Engage patients in their healthcare journey, establish meaningful connections and trusting relationships with each patient and provide care tailored to the patient.
  • Deliver exceptional customer service to our patients, caregivers, and clinical partners including proactive, timely, and comprehensive communication from a holistic patient-centered perspective.
  • Maintain adherence to clinical guidelines, policies, and best practices, ensuring efficient and effective care delivery.
  • Participate in team huddles and meetings.
  • Comply with all relevant healthcare laws, regulations, and company standards.
  • Other duties as assigned.

Job Specifications

  • Active, unrestricted RN license in the state where you practice
  • At least 1 year of nurse leadership experience in a supervisory or lead position.
  • At least 2 years of direct patient care, preferably in an ambulatory setting.
  • Current certification in Basic life support (BLS).
  • Proficiency in blood draws, laboratory specimen collection, medication and vaccine administration, urinary catheterization, wound care, removal of sutures/staples, basic hearing and vision tests, and home safety evaluations. Able to coach and provide guidance.
  • Training and/or experience in complex care, geriatrics, and/or palliative medicine
  • Ability to conduct independent home visits.
  • Willingness to travel 75% of the time within assigned area.
  • Ability to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required.
  • DRIVER REQUIREMENTS: Licensed for a minimum of 5 years. No vehicle-related suspensions/reinstatements, DUI, reckless driving, or felony convictions within the last 7 years. Multiple violations and accidents within the last 5 years would be subject to review.

Specialized Knowledge And Skills

  • Ability to train and evaluate clinical skills and workflows against organizational and practice standards.
  • In-home care delivery practice standards.
  • Knowledge of nursing, medical assistant, and paramedic/EMT practice standards, including safe and effective delegation practices.
  • Familiarity with current infection prevention & control resources aligned with regulatory standards (e.g., OSHA, CDC, and AHRQ).
  • Proficient in Microsoft Office Suite (Excel, Word, PowerPoint).

Base Salary/ Wage Range $95,000 to $100,000/yearly with annual bonus. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown. ConcertoCare offers a competitive total rewards package, which includes full healthcare coverage, a 401K with match, and a broad range of other health, wellness, and financial benefits.
