Summary of Principal and Major Responsibilities:
This position will directly support the Sisters of the Congregation of Divine Providence. The candidate will work closely with the Superior General and General Counsel to determine a specific schedule, as this professional role may require availability on weekends and evenings. The scope of the position may expand to include direct care responsibilities at Regan Hall.
The Wellness Coordinator will be an integral part of the Health and Wellness team. In collaboration with this team, the Coordinator will assist Sisters in making informed health-related decisions, provide resources, and facilitate access to necessary services. The Coordinator will carry out these responsibilities in alignment with the Wellness Plan and will serve as the primary liaison for the Coordinating Team and Wellness Plan. They will be accountable to both the Coordinating Team and the Counsel in conjunction with the Wellness Team.

Organization Duties and Responsibilities:

_ Principal Duties and Responsibilities (Essential Functions):_
_ Develop and Implement Wellness Plans:_
- Develops and implements individualized Wellness Plans for Sisters of the Congregation of Divine Providence that address their stage of life, health-related needs, and risk factors. Ensures that these plans align with CDP and community goals and values, and provide appropriate services to meet their needs.
- Develops and identifies existing formation and education programs on Health and Wellness.
- Develops and implements processes and systems to continuously assess the health needs of the Sisters, and regularly updates and reviews Wellness Plans.
Assess Ongoing Healthcare Needs (requirements):
- Assists and advises Sisters regarding healthcare decisions, identifies resources, facilitates referrals, and coordinates related services.
- Maintains regular contact, prioritizing those who are needing special care and support.
- Offers compassionate listening.
- Maintains and updates records as changes occur.
- Identifies and provides recommendations to supervisor when the need for a higher level of care is required.
- Assesses needs and advocates for Sisters during acute care episodes.
Utilize Support Services:
- Collaborates closely with the supervisor on special orders for equipment, medications, and medical supplies.
- Collaborates with other care providers supporting Sisters as part of the care team and helps identify resources that can offer additional expertise and support.
- Delivers care management services according to their Wellness Plans including referral. Referrals to health and wellness professionals, coordinating access to home and community-based services, determining appropriate levels of care, and offering ongoing support.
Compassionate Care & Education:
- Provides support and guidance to Sisters in making lifestyle changes, managing disease, coping with chronic illness, and addressing other health concerns, or assisting during times of crisis.
- Although this role primarily functions at a high level, there will be occasions when the Health and Wellness Coordinator is expected to accompany a Sister to medical appointments, be present in the emergency room, or support a Sister during treatments, surgeries, coordinate discharge planning, or other health-related events.

Physical and Position Requirements:
- Ability to sit, stand, and walk, with or without reasonable accommodation.
- Able to push, pull, lift, and/or carry up to 15 pounds to assist Sisters with transportation (e.g., getting in and out of vehicles, pushing a wheelchair).
- Manual dexterity necessary for performing health assessments.
- Sufficient vision to read reports and use computers, with or without reasonable accommodation.
- Ability to read, comprehend, reason, interpret, and analyze information effectively.
- Strong competency in written communication.
- Must be able to read and understand English for tasks such as reading materials or ordering supplies.
- Adheres to CDP, Inc. policies and procedures, including safety and health guidelines.
Knowledge, Skills and Abilities:
- Training and hands-on experience in healthcare, particularly in supporting individuals with chronic health conditions and the elderly;
- Aligned with and supportive of the Mission and Values of the Congregation of Divine Providence;
- Demonstrated knowledge of:
- Health care systems that include acute care, home and community-based services and long-term care;
- Person-directed and holistic models of care;
- Principles of advocacy;
- Skilled in researching community resources and sharing information with others as needed;
- Able to build and sustain collaborative relationships within the congregation and with various healthcare professionals;
- Able to communicate professionally and compassionately with Sisters who may not request or recognize the need for support or intervention;
- Experience working with religious congregations is a plus.
- Familiarity with the values and traditions of religious communities is preferred.
- Proficient in using Internet Explorer, Microsoft Office, and other relevant professional software programs as needed.
Work Environment:
- May be On-Call.
- Exposure to risks typical of both a standard office setting and an eldercare facility environment.
- Potential or direct exposure to blood, bodily fluids, infectious waste, diseases, and conditions (e.g., TB, Hepatitis B).
_ Education and Experience:_
- RN, or LVN with experience in long-term care or assisted living.
- Social work experience or a licensed social worker with expertise in healthcare or eldercare is also considered.
- Preferred experience in long-term care or home health.
- Minimum of 3-5 years of care management experience.
- Degree in Health Administration, or a degree combined with additional certifications related to health and wellness.
Licensure/Certification:
- Current license required for RN/LVN.
- Must have a valid and current driver’s license and consent to a Department of Motor Vehicles review of past driving record to confirm safe driving status.
- Preference is given to candidates who are licensed professionals with a degree in a health-related field.

Pay: $57,000.00 - $65,000.00 per hour
