We are seeking a dedicated and experienced Clinical Coordinator to join our team. This individual will play a pivotal role in ensuring the delivery of exceptional care while supporting our clinical staff and adhering to compliance standards.

Location: American Fork, UT
Responsibilities:
- Monitor and evaluate clinical processes to ensure compliance with state, federal, and accreditation standards.
- Conduct audits of patient charts, documentation, and care plans to ensure accuracy and adherence to regulations.
- Collaborate with the clinical team to develop and implement quality improvement initiatives.
- Track, analyze, and report quality metrics to identify trends and opportunities for improvement.
- Ensure readiness for regulatory and accreditation surveys.
- Collaborate with our team to develop, implement and evaluate quality, programs and other ways to continue providing the best care to our patients.
- Review clinical assessments completed upon admission, hospitalization, evaluation, and discharge (OASIS & HIS data).
- Review clinical notes, ensuring the best practices.
- Provide training and support for clinical team members as needed.
- Stay up-to-date on clinical programs, policies, processes and changes
- Interact well with physicians, discharge planners, patient, family, team members and other healthcare professionals.
- Provide direct patient care on occasion, as necessary, to support the team.

Licenses Required:
- Active Registered Nurse (RN) license in Utah
- Current CPR certificate
- Valid Utah driver license with car insurance
Job Type: Full-time
Pay: $70,000.00 - $80,000.00 per year
