The Person-Centered Options Counselor (PCOC) is an individual who promotes and encourages independence for all people with disabilities. The position provides services and support for individuals in the community exploring long-term supports and services and medical coverage through the State Health Insurance Partnership program (SHIP)

· Responds to referrals & requests for services

· Provides appropriate and timely referrals to community resources and provides follow-up to ensure services meet the needs of individuals served.

· Assists individuals in navigating long-term community support and services tailored to their personalized needs and preferences.

· Conducts comprehensive assessments with individuals to target their specific needs, desires, and goals.

· Assists individuals with goal setting and developing an action plan to meet their independent living desires

· Provide unbiased counseling related to healthcare coverage, possible subsidy eligibility, Medicare preventative services, and prescription drug plans.

· Assists with program related outreach and education initiative designed to promote PCOC/SHIP and build connections within the community.

· Work alongside No Wrong Door (NWD) Program Manager to achieve the goals of the NWD program.

· Has regular contact with community members via telephone, email and/or in person

· Prepares written reports and billings as required by funding source

· Ensures that services and activities are implemented with strict adherence to the DNMM Code of Ethics, governmental and contractual requirements, and the Independent Living

Philosophy.

· Maintains good working relationships with contracting agencies and community partners

· Maintains consumer data and financial records as required by contracting agencies, grant funders, and the law

· Attends DNMM Team meetings and community meetings as assigned by Team Leader

· Attend Initial and refresher training courses as required.

· Promote and encourage independence for all people with disabilities

· Maintain all files and complete all documentation

· Complete paperwork such as travel, time sheets, and reports

-Knowledge of community resources including options for long-term support and services.

-Knowledge of HIPAA compliance.

-Familiarity with Mandated Reporter guidelines.

-Knowledge and application of Person-Centered Processes.

-Knowledge of and experience with developmental disabilities and mental health disabilities.

-Familiarity with scheduling/tracking of due dates.

-Knowledge of community resources and disability issues

-Computer skills.

-Excellent written and oral communication skills.

-Problem solving and organizational skills.

-Time management skills.

-Respect for people with disabilities.

-Promote consumer choice.

-Ability to work independently.

-Ability to maintain consumer confidentiality.

-Ability to locate suitable resources as requested by the public.

No supervisory responsibilities are associated with this position

· Documentation within 24 hours

· Completion of above standards by month end of each month

· Work with individuals in the environment of their choice

· Follow Mandated Reporter guidelines

High School Diploma or G.E.D.

Bachelor’s Degree in Health & Human Service related field, equivalent life experience, or CHW certification preferred

· Knowledge of and sensitivity to disability issues and an understanding of advocacy on an individual and community-wide basis.

· Demonstrated skills in oral and written communication, with or without accommodation.

· Demonstrated understanding of and commitment to Independent Living Philosophy.

Employees are expected to be available during DNMM business hours from 8:00 am to 4:30 pm, and to work the authorized hours of this position each week. Flexibility in scheduling work outside of regular business hours may occasionally be necessary.

· Travel to and from community locations within a 12-county radius is expected.

· Entering individual homes and/or community locations where steps may be present

· Carrying/moving items, climbing stairs

This job typically operates in a professional office environment, routinely using standard office equipment such as computers, phones, photocopiers as well as fax machines and filing cabinets.

Additional work settings may be required depending on the type of work performed such as:

· Regular travel, often to low-income neighborhoods

· Ability to work in higher risk, non-accessible environments

Travel to and within community locations within the boundaries of DNMM’s 12 county catchment area (Alcona, Arenac, Bat, Clare, Gladwin, Gratiot, Iosco, Isabella, Midland, Ogemaw, Roscommon, Saginaw) is expected about 35-40% of the time
